Resource Description:

The goal of the Single Entry Point (SEP) program is to provide long-term care services to elderly and disabled persons  as a means to help them continue to live in community settings as alternatives to nursing home care.  SEP services the following populations:

  • Elderly, blind and disabled
  • Persons with major mental illness
  • Persons with brain injury
  • Children with life limiting illness

Eligibility Requirements:

  • Must meet a Functional Assessment criteria
  • Be Medicaid eligible
  • Eligibility and service needs are re-determined on an annual basis or at a change in circumstance
