Resource Description:
The goal of the Single Entry Point (SEP) program is to provide long-term care services to elderly and disabled persons as a means to help them continue to live in community settings as alternatives to nursing home care. SEP services the following populations:
- Elderly, blind and disabled
- Persons with major mental illness
- Persons with brain injury
- Children with life limiting illness
Eligibility Requirements:
- Must meet a Functional Assessment criteria
- Be Medicaid eligible
- Eligibility and service needs are re-determined on an annual basis or at a change in circumstance
